When a questionnaire is created it is automatically activated. Questionnaires cannot be deleted, they can only be deactivated. Deactivating a questionnaire does not remove its association with any requisitions.
To deactivate an existing questionnaire
Go to Primary Navigation Bar > Admin > Questionnaires.
Click the Questionnaires tab to view the Questionnaires page.
Locate the questionnaire.
Select Deactivate from the Actions drop-down list. A message appears confirming the questionnaire was deactivated and a red X appears next to the questionnaire on the Questionnaires page.
You can also deactivate questionnaires in bulk by selecting the check box in the header row and selecting Deactivate from the Actions drop-down list.
