Create a Questionnaire
Once you have created several questions, you can build a questionnaire with those questions. Creating a questionnaire template requires three major steps:

Complete basic Questionnaire General Information—including code, name, and type.

Assign Questions to be included in the questionnaire.

Configure settings specific to the questionnaire, such as additional instructional text, required questions, etc.
Once a questionnaire is created you can assign it to a requisition. For more information on assigning questionnaires to a requisition see, Assign Questionnaires to an Existing Requisition.
To create a questionnaire
- Go to .
- Click the Questionnaires tab to view the Questionnaires page.
- Click Create Questionnaires. The Create Questionnaire dialog box displays.
- Enter a unique alphanumeric value in the Code field for this new questionnaire. The unique code is used in reporting. It cannot exceed 50 characters.
- Enter the Internal Name for the questionnaire. This name will only appear in PeopleFluent Recruiting Portal and the PeopleFluent Administration. If your company supports more than one language, click the Globe icon and enter the Internal Name for each language.
- Enter the Public Name that candidates will see when completing the questionnaire. If your organization uses more than one language in PeopleFluent Career Sites, click the Globe icon
and type the Public Name in each supported language.
- Enter a Description of the questionnaire for internal users.
- Select a Category from the list. Associating the questionnaire with a category makes it easier to search for the questionnaire.
- Select the Available for Use in the Recruiting Portal check box to make the questionnaire available to add to a requisition in PeopleFluent Recruiting Portal.
- Click Add Question and select a series of questions to the questionnaire. If your organization supports multiple locales, use the language drop-down list to toggle preview between supported languages. When adding questions
to a questionnaire, you can do the following:

Make a question required in the questionnaire.

Change the order of the questions appear on the questionnaire by using a drag and drop operation.

Move a question to the top or bottom of the questionnaire.

Delete a question from the questionnaire.

Insert a page break between questions.

Click Save.
